Pelvic and Acetabular Overview

Pelvic Injuries

The pelvis is the ring-shaped bone at the base of the spine. Pelvic injuries occur when one or more bones of this ring are fractured. These injuries can range from mild, stable fractures to severe, unstable injuries that require urgent treatment.

Acetabular Injuries

The acetabulum is the “socket” of the hip joint. An acetabular fracture occurs when the socket breaks—often due to high-impact trauma like road accidents or falls. This type of fracture may affect hip stability and joint function.

Causes

  • Road traffic accidents

  • High-energy falls

  • Sports injuries

  • Osteoporosis-related fractures (in elderly)

  • Industrial or crush injuries

Common Symptoms

  • Severe hip or pelvic pain

  • Difficulty standing or walking

  • Swelling or bruising around the hip/pelvis

  • Leg appearing shorter or rotated

  • Inability to bear weight

  • Pain on movement of the hip

Causes

  • Road traffic accidents

  • High-energy falls

  • Sports injuries

  • Osteoporosis-related fractures (in elderly)

  • Industrial or crush injuries

Treatment Options

1. Non-Surgical Treatment

Used for stable fractures:

  • Bed rest

  • Pain management

  • Walking aids

  • Physiotherapy

2. Surgical Treatment

Required for unstable or displaced fractures:

  • Pelvic fracture fixation

  • Acetabular fracture fixation

  • Minimally invasive or open techniques

  • Hip replacement (in severe acetabular damage or elderly patients)

Surgery aims to restore the pelvic ring, rebuild the hip socket, and prevent long-term complications like arthritis.

Recovery

  • Weight-bearing is introduced gradually

  • Physiotherapy is essential for mobility and strength

  • Healing typically takes 8–12 weeks, depending on the injury

Potential Complications

  • Post-traumatic arthritis

  • Hip stiffness

  • Nerve or vessel injury (rare)

  • Limping if not treated properly

  • Malunion or non-union of the fracture

Early evaluation by a specialist helps prevent long-term disability.
