Hip AVN (Avascular Necrosis of the Hip) Overview

Avascular Necrosis (AVN) of the hip—also known as Osteonecrosis—is a condition in which the blood supply to the head of the femur (hip ball) is reduced or completely cut off. Without proper blood flow, the bone becomes weak, collapses, and leads to severe arthritis.

It commonly affects young and middle-aged adults and can progress rapidly if not treated early.

Causes of Hip AVN

Hip AVN can occur due to:

  • Trauma (hip fracture or dislocation)

  • Long-term steroid use

  • Alcohol overuse

  • Blood disorders (e.g., sickle cell disease)

  • Autoimmune diseases

  • Post-COVID steroid treatments

  • Idiopathic (unknown cause) – in many patients

Common Symptoms

  • Deep groin or hip pain

  • Pain during walking, running, or climbing stairs

  • Night pain

  • Reduced hip movement

  • Limping

  • Sudden worsening when bone collapse begins

Treatment Options for Hip AVN

1. Early Stages (Before the Bone Collapses)

  • Medication to improve blood flow

  • Activity modification

  • Bisphosphonates (in selected cases)

  • Physiotherapy

  • Core Decompression Surgery
    – reduces pressure inside the bone
    – stimulates new blood vessel formation

  • Stem Cell–Augmented Decompression (in some centers)

2. Advanced Stages (Bone Collapse or Severe Pain)

Once the femoral head collapses, joint-preserving procedures are less effective.

The most reliable treatment becomes:

Suitability also depends on patient anatomy, body type, and severity of disease. Your orthopaedic surgeon will determine if DAA is ideal for you.

Total Hip Replacement (THR)

Modern hip replacement—especially DAA, Bikini Incision, or Robotic Hip Replacement—offers:

  • Excellent pain relief

  • Restoration of mobility

  • Long-term implant survival

  • Faster recovery and early walking

Hip replacement for AVN patients generally gives outstanding long-term results, even in younger individuals.
